All of these forms are Adobe PDF fill-in forms. You can either print the blank form and fill in the information by hand, or you can type the information right into the form then print it. To type information right into the form:

Click the hand tool on your tool bar and click into the first field.
When you see the blinking cursor, you can start typing in the information into that field.
Simply hit the tab key to move between fields.
To clear all of the fields in the application, click the "Clear Form Fields" button at the bottom of the form. WARNING: This will erase all of the information you have entered in the form.
When you are finished entering information, click the printer icon on your toolbar to print.
