Dental Benefits Information

Patients without dental benefits:

If you do not have dental insurance, we request payment at time of service, unless special financial arrangements have been previously made.

Patients with dental benefits:

If you do have dental benefits, please provide us with your dental benefit information as soon as possible so that our expert front office staff can check your benefits and send in the proper claims for reimbursement.

As a courtesy, only the estimated portion of your bill not covered by insurance will be due at the time of service. Your portion is due at the time of service. If your portion cannot be paid at the time of service, we will discuss other financing options with you prior to treatment. However, regardless of your dental coverage, you are responsible for the entire account balance.
